What do Peer Support Specialists do? Lots! ​
​
We like to think of them as the ultimate community care providers meeting people and families where they are, They are compassionate mentors, attentive listeners, warm huggers (when that is safe to do again) and all around good humans.
​
Services are individualized and mutually agreed upon with program members. Peer Support Specialists are the backbone of our Revive to Thrive Program providing much needed personal support and linkage to resources.
Services include:

Peer Recovery Coaching & Recovery Planning

Resource navigation - warm handoffs and referrals to other appropriate resources, conducted bypeer specialists

Overdose prevention: No-cost naloxone kits and training
-
Post overdose response team: two team members go out and visit individuals 24-72 hours after
a reported overdose to offer harm reduction, counseling, and treatment.

Harm reduction counseling on avoiding blood-borne diseases and overdose, and safer use
practices

Disease testing through our partnership with Nova Salud

Relapse prevention and recovery support - individual or group meetings for one year with a peer
specialist

Medication: assistance with assessment and induction on medication as appropriate

Follow up care - monthly check-ins with a peer specialist

Transportation to appointments and services if qualified.
Application assistance provided

Substance Use Disorder or Mental Health treatment referrals with warm handoffs and enrollment support

Health insurance navigation/ to Medicaid, ACA, SSI

SNAP or food pantry resources
-
Housing aid: funding for a nonrefundable move-in fee and first two weeks rent in a recovery
residence, for those who qualify. Resources to other organizations that provide housing
assistance.

Employment/transportation: assistance in job search, resume building, transportation services,
and resources

Legal identification documents: (Driver license, SS card, Birth Certificate)
