Revive to Thrive

A Community Driven Continuum of Care

The CAF provides a spectrum of services across all stages of substance use disorder from active addiction to

long-term recovery.

Trained peer support specialists walk  program members through an individualized plan of services appropriate for their needs and according to their mutually determined health goals.

Services 
  • Overdose prevention - FREE naloxone kits and overdose reversal training 

  • Harm reduction counseling on avoiding blood borne diseases and overdose, and safer use practices 

  • Disease testing through our partnership with Nova Salud

  • Treatment referrals with warm handoffs and enrollment support 

  • Assistance enrolling in healthcare 

  • Recovery support - weekly one-on-one meetings (in first year) with a Peer Support Specialist

  • Housing aid - funding for non-refundable move-in fee and first two weeks of rent at a recovery residence, oxford house, or sober living

  • Employment assistance

  • Transportation assistance – rides to appointments, job interviews, etc. 

  • Resource navigation - warm handoffs and referrals to other appropriate resources

  • Follow up care - monthly check ins (in year two) with a Peer Support Specialist 

  • Family Education Groups and materials for the family of members

  • Linkage and scholarships to online family education academy 

  • Linkage to private family recovery coach
